Immunoglobulin D (IgD) is an antibody isotype that makes up about 1% of proteins in the plasma membranes of immature B-lymphocytes where it is usually co-expressed with another cell surface antibody called IgM.
The diagnosis is probable when blood tests show the complete lack of circulating B cells (determined by the B cell marker CD19 and/or CD20), as well as low levels of all antibody classes, including IgG, IgA, IgM, IgE and IgD.
All B cells begin by co-expressing antibodies that have IgM and IgD constant regions, but they are later able to exchange these constant regions for IgA, IgG or IgE constant regions and express antibodies of a different class type via class switch recombination.
Naive B cells express IgM and IgD isotypes with unmutated variable genes, which are produced from the same initial transcript following alternative splicing.
B1 cells characteristically express high levels of surface IgM (sIgM), demonstrable CD11b, and low levels of surface IgD (sIgD), CD21, CD23, and the B cell isoform of CD45R (B220).
FO B cells express high levels of IgD, and CD23; lower levels of CD21 and IgM; and no CD1 or CD5, readily distinguishing this compartment from B1 B cells and marginal zone B-cells.
