- The pudendal canal that carries the pudendal nerve is also known by the eponymous term "Alcock's canal", after Benjamin Alcock, an Irish anatomist who documented the canal in 1836.
- It divides into the labia majora (literally "larger lips"), on either side of the furrow known as the pudendal cleft that surrounds the rest of the vulvar parts: labia minora, clitoris, urinary meatus, vaginal opening, and vulval vestibule.
- The incision, which can be done from the posterior midline of the vulva straight toward the anus or at an angle to the right or left (medio-lateral episiotomy), is performed under local anesthetic (pudendal anesthesia), and is sutured after delivery.
- A 2009 review study found both that "prevalence of PN is unknown and it seems to be a rare event" and that "there is no evidence to support equating the presence of this syndrome with a diagnosis of pudendal nerve entrapment," meaning that it could be possible to have all the symptoms of pudendal nerve entrapment (otherwise known as pudendal neuralgia) based on the criteria specified at Nantes in 2006, without having an entrapped pudendal nerve.
- In both sexes, they are innervated by the deep or muscular branch of the perineal nerve, which is a branch of the pudendal nerve.
- The pudendal block gets its name because a local anesthetic, such as lidocaine or chloroprocaine, is injected into the pudendal canal where the pudendal nerve is located.
- The pudendal nerve runs between the coccygeus muscle and the piriformis muscle, superficial to the coccygeus muscle.
- The pudendal nerve passes on the lateral surface of the internal obturator muscle and the coccygeus muscle.
- The sacral plexus is formed anterior to the piriformis muscle and gives rise to the sciatic nerve, the superior and inferior gluteal nerves, and the pudendal and posterior femoral cutaneous nerves.
- Instead of piercing the sacrotuberous ligament, the perforating cutaneous may instead run with the pudendal nerve, or pass in between the sacrotuberous ligament and the gluteus maximus muscle.
- The pudendal vessels and nerve pass behind the sacrospinous ligament directly medially and inferiorly to the ischial spine.
- The perineal artery (superficial perineal artery) arises from the internal pudendal artery, and turns upward, crossing either over or under the superficial transverse perineal muscle, and runs forward, parallel to the pubic arch, in the interspace between the bulbospongiosus and ischiocavernosus muscles, both of which it supplies, and finally divides into several posterior scrotal branches which are distributed to the skin and dartos tunic of the scrotum.
- The inferior rectal artery arises from the internal pudendal artery as it passes above the ischial tuberosity.
- The pudendal canal (also called Alcock's canal) is an anatomical structure formed by the obturator fascia (fascia of the obturator internus muscle) lining the lateral wall of the ischioanal fossa.
- The pudendal nerve arises from spinal roots S2 through S4, travels through the pudendal canal on the fascia of the obturator internus muscle, and gives off the perineal nerve in the perineum.
- The inferior rectal nerves (inferior anal nerves, inferior hemorrhoidal nerve) usually branch from the pudendal nerve but occasionally arises directly from the sacral plexus; they cross the ischiorectal fossa along with the inferior rectal artery and veins, toward the anal canal and the lower end of the rectum, and is distributed to the sphincter ani externus (external anal sphincter, EAS) and to the integument (skin) around the anus.
- The notch holds the piriformis, the superior gluteal vein and artery, and the superior gluteal nerve; the inferior gluteal vein and artery and the inferior gluteal nerve; the sciatic and posterior femoral cutaneous nerves; the internal pudendal artery and veins, and the nerves to the internal obturator and quadratus femoris muscles.
- It is converted into a foramen, the lesser sciatic foramen, by the sacrotuberous and sacrospinous ligaments, and transmits the tendon of the obturator internus, the nerve which supplies that muscle, and the internal pudendal vessels and pudendal nerve.
- The deep external pudendal artery (deep external pudic artery) is one of the pudendal arteries that is more deeply seated than the superficial external pudendal artery, passes medially across the pectineus and the adductor longus muscles; it is covered by the fascia lata, which it pierces at the medial side of the thigh, and is distributed, in the male, to the integument of the scrotum and perineum, in the female to the labia majora; its branches anastomose with the scrotal or labial branches of the perineal artery.
- After piercing the femoral sheath and fascia cribrosa, it courses medialward, across the spermatic cord (or round ligament in the female), to be distributed to the integument on the lower part of the abdomen, the scrotum in the male, and the labium majus in the female, anastomosing with branches of the internal pudendal artery.
